How the number reads
Numbering does not have to begin at 1 — a report whose first section ended at 4 can continue at 5. The "of M" total counts the final number, so starting at 5 on a three-page document reads "Page 5 of 7".
Where it sits
Eighteen points in from the chosen edge — the same margin on every page.
The numbers are stamped onto the page as ordinary text: real ink, not an overlay a reader can switch off, and not something that can be un-numbered later without the original file.
